Hey peeps, what's up? I'm currently conducting survey questionaires for an upcoming possible "Family Feud" type game show in about 2-3 months. I will be the host of the show by the way. the surveys are conducted in the Louisville, Kentucky/Southern Indiana area. I was wondering if there is a program where you make your own surveys and have a "limit," meaning that I want to survey 100 people, and also have people type in their answers, have it recorded, etc? I was hoping for such a program like that.
So far, I've conducted 40 questions so far and have 3 sets (1st & 2nd set--15 questions each and 3rd set--10 questions) I have 15 people responded to the set 1 questions, 20 people responded to set 2, and 1 people responded to set 3.
Also, I am doing a format (single-single-single-double-triple-triple [sudden death]) with the first team to reach 400 points.

I don't understand what you're asking for. Are you asking for a program that installs on a single computer or a program that works online? If you want something online, you're in the same boat I'm in. I'm working to develop a way of surveying people from my web page.
If you're looking for a standalone program, you're not going to find it. Somehow, I can't imagine a software writer sitting down to write a program that meets your requirements. Then again, no one imagined a programmer writing Family Feud either.
I might could do something with Flash but two problems arise from that. One, I don't know what exactly you're looking for. Two, it'll push back other projects such as Family Feud Challenge, $100,000 Pyramid, and the unannounced Match Game.
